The steel begins to rust and as it rust more and more it begins to expand and push out the concrete and over time the pressure builds up more and more until the concrete gives and starts to crack on the surface. Once this starts to happen the crack will continue to grow and grow and simply filling in the crack will not prevent further damage or fix the problem.
